Leg length discrepancy (LLD) occurs when one leg is shorter than the other. This condition can be congenital or develop later due to injury or illness. Minor differences in leg length typically don’t cause issues. However, for the small percentage of individuals with a discrepancy greater than 1 cm, the difference can impact their overall well-being and quality of life.
Types of Leg Length Discrepancy
Structural Discrepancy:
Bone Length Difference: This type involves a physical difference in the length of the bones in each leg.
Causes: Congenital disorders, developmental anomalies, growth plate injuries, certain bone diseases or infections, and previous trauma or surgery.
Functional Discrepancy:
Postural or Muscular Imbalance: Problems with posture or alignment of the spine and pelvis can create a perceived discrepancy, rather than an actual difference in bone length.
Causes: Poor posture, muscle tightness or imbalances, scoliosis, or joint alignment issues.

Signs and Symptoms of Leg Length Discrepancy
Uneven Legs: A visible difference in leg length when standing or lying down.
Posture Changes: Asymmetrical foot posture (one foot appearing more collapsed or high-arched), one knee appearing more bent, uneven shoulders, or a tilted pelvis.
Gait Issues: Limping or uneven walking resulting from compensatory adjustments.
Pain: Discomfort in the lower back, hips, knees, and lower legs due to altered gait.
Wear Patterns: Uneven wear on shoes caused by a changed walking pattern.
Treatment and Management of Leg Length Discrepancy
Custom Foot Orthotics: Insoles or lifts used to balance leg length and minimize compensatory adjustments.
Footwear Modifications: Adding a full-length lift to the base of the shoe for additional correction.
Strengthening and Stretching Exercises: To reduce compensatory movements, improve posture, and enhance flexibility.
Surgery: In severe cases, surgical procedures may be required to correct the affected leg.
